Special discount rate will be provided to the Affiliate Fellows of The Hong Kong College of Community Medicine. If you would like to join as new Affiliate Fellow to enjoy discount rate for the ASM and free admission fee for your application as the Affiliate Fellow, you are required to (1) download and complete the application form HERE; and (2) prepare a cheque at HK$275 for Annual Subscription Fee.

Please mail the application form together with the cheque made payable to "Hong Kong College of Community Medicine" to the following address 2 weeks within your registration:
